PBHS HEALTH CARD CONDITIONS
1.On joining any COA of our company discount will be provided on health facility and if anybody needs Health Card then it will be made available in Rupees 25/- only (Within 15 minutes).

2.On completion of 3rd COA Rupees 100 should be paid by you or will be deducted from your wallet. Till the amount will not be paid by you your services of health card will be adjourned and as soon as the amount will be paid your services will be started.
3.On completion of 4th COA Rupees 200 should be paid by you or will be deducted from your wallet. Till the amount will not be paid by you your services of health card will be adjourned and as soon as the amount will be paid your services will be started.
TERMS & CONDITIONS
1.Customer can use health card benefits if he/she will convert his/her coupons time to time provided by company in customers in 24 hours if not the health service card benefits will be adjourned and after converting coupons into customers the health card service will be started again.
2. According to the up gradation of COA the health card will get upgraded.
3.Validity of health card will be of 90 days.
4.At the time of reaching the 5th COA if he/she does not purchase the health card then his/her card will be upgraded by the company and for that he/she have to pay the amount of Rupees 150 per month. Otherwise this amount will be deducted according to per day calculation from his/her wallet.
